In order to obtain any result first check on Carnet Santé Québec
The Carnet Santé Québec allows:
Access to the list of medication on your file;
Access to reports of your medical imaging tests (ultrasound, X-ray, etc.)
Access to your blood test results, urine tests and other samples.

If you have not received your result after 1 month or if you would like to discuss about them with your doctor please contact our secretary. Please note that there may be a delay before the result is available. Your doctor needs to read it beforehand and communicate to you the results. Please note the doctor must also give his authorization before a result is communicated to the patient by the secretary.
If you need to have a form (driver's license, insurance, etc.) completed by your doctor:
Please submit your form to the reception, while making sure you:
Indicate the name of the doctor concerned by your request;
Indicate your full name;
Sign the document or form (if applicable)
Please note that there will be a fee for forms not covered by the Régie de l'assurance-maladie du Québec.
It is essential to bring the documents as soon as possible and to allow a delay in processing your request.
You will be contacted when the form or document has been completed.

A medical reassessment is generally recommended at the time of medication renewal. You should therefore schedule an appointment with your doctor beforehand.
Did you know that it is possible to ask your pharmacist to send a renewal request by fax to your family doctor, especially when there is a list of several medications to be renewed.
The following is now possible upon request:
The pharmacist may send a medication refill request to your family doctor;
The doctor takes note of the request;
If approuved, the doctor prepares a prescription and sends it to your pharmacist;
You receive a call from the pharmacy when your medications are ready.
You may ask your pharmacist to fax a medication renewal request to your family doctor.
Your doctor may first request a medical visit if he deems it necessary
Please be sure to make any request in advance.
Your community pharmacist can renew your medications

If you are unable to find your results after 1 month or if you wish to discuss your results with your doctor, please contact the secretariat.
For any new request, you must make an appointment with your family doctor.
For a copy of a request or lost document, a fee may be required.
To obtain a referral to a specialist or any other healthcare professional, you must make an appointment with your family doctor discussing the referral.
A doctor's assessment is required prior to any referral.
If your condition changes or if the expected time is exceeded, please make an appointment with your doctor for a follow-up.
The waiting time for an appointment with a specialist varies on the specialist, the waiting list and the level of priority.
